29 CFR 1910.217(f)(2): Operator(s) were not trained and instructed in the safe method of work before starting work on mechanical power press(es). The employer did not ensure by adequate supervision that correct operating procedures were being followed: Employees were exposed to amputation hazards. Employees engaged in 60-Ton Torq-Pacs mechanical power press operation activities in the small punch press shop of building #6 were exposed to amputation hazards in that the training in the safe method of work, machine operation and set up procedures was inadequate. The employer did not ensure by adequate supervision that correct operating and set up procedures were being followed, which caused the equipment to jam at various stages of the machine cycle. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.217(h)(13)(ii): The employer did not certify that employees had been trained by preparing a certification record which included the identity of the person trained, the signature of the employer or the person who conducted the training, and the date the training was completed: Employees were exposed to amputation hazards. Employees engaged in 60-Ton Torq-Pacs mechanical power press operation activities in the small punch press shop of building #6 were exposed to amputation hazards in that a certification record of training was not implemented and maintained. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
