29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. On or about February 17, 2021, employees were exposed to a struck by hazard while performing sewer line replacement at 3500 Medina Road, Medina, Ohio 44256. An employee was in a trench that was approximately 9 feet deep and the employer did not ensure that all tools and materials at the top of the trench were at least two feet back from the edge.
