29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  Grant Grove Maintenance Facility:  The Clark (AM5V45) power industrial truck was being used with an unauthorized personnel hoisting basket. Employees were exposed to crushing injuries.

The U.S. National Park service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(3), which was contained in OSHA inspection number 1164977, notice number 02, item number 001 and was affirmed as a final order on 10/06/2016, with respect to a workplace located at Grant Grove, 83715 Park Road, Kings Canyon, CA 93633.

NP8000021032) work rest was not adjusted to 1/8th of an inch. The opening was half of an inch when measured.  Employees were exposed to struck-by/caught-in hazards.
