29 CFR  1910.244(a)(2)(i):In the absence of a firm foundation, the base of the jack shall be blocked. If there is a possibility of slippage of the cap, a block shall be placed in between the cap and the load.  a) Dallas Trailer Repair Co., Inc. at 8951 Yosemite Street, Henderson, CO 80640:   On February 14, 2021, the employer did not ensure that the cap of Emerson Jack, Model 110 was secured under the load in order to prevent slippage.   Employee was attempting to lift a tandem axle on 53-foot semi-trailer when the 5.5 inch x 2.0 inch jack cap slipped out from under the cylindrical axle, causing the jack handle to strike the employee in the face.  Employee sustained facial fractures and injury to the right eye as a result.  This condition exposed employees to struck-by and crushed-by hazards.
