29 CFR 1910.333(a)(2): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards:   (a) On or about February 13 , 2021, employees performing electrical work on or around 13,2000 volt AC energized electrical circuits and equipment, did not employ electrical safety-related work practices  while working on the Russelectric  15,000 volt ATS #2 circuit breaker.  This exposed employee(s) working at or around the circuit breaker to Arc-flash, electrical shock, and burns.

29 CFR  1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on were not disconnected from all electric energy sources:   (a) On or about February 13, 2021,  employee(s) performing electrical work on or around the Russelectric 15000 volt ATS #2 circuit breaker did not deenergized and disconnect the circuit breaker from the 13,200 volt AC  energy source supplying the energy to the unit, when employee(s) opened the circuit breaker panel door to inspect the inside electrical components.  This exposed employee(s) working at or around the circuit breaker to Arc-flash, electrical shock, and burns.  In the alternative...   29 CFR  1910.147(c)(4)(i): Procedures were not  utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about February 13 , 2021, employees  performing electrical work on or around the Russelectric  15000 volt ATS #2 circuit breaker did not utilize procedures to control hazardous electrical energy when employee(s) opened the circuit breaker panel door to inspect the inside electrical components.  This exposed employee(s) working on or around the circuit breaker to Arc-flash, electrical shock, and burns.

29 CFR  1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed.  (a) On or about February 13, 2021,  employee(s) performing electrical work on or around the Russelectric 15,000 volt ATS #2 circuit breaker did not test the electrical circuit to insure that it was in a de-energized state prior to conducting work on the 13,200 volt AC circuit.  This exposed employee(s) working at or around the circuit breaker to Arc-flash, electrical shock, and burns.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:  (a) On or about February 13, 2021, employee(s) working on and around an energized Russelectric 15,000 volt ATS #2 circuit breaker, supplied by a 13,200 volt AC circuit, did not use personal protective equipment, such as, but not limited to arc rated apparel, faceshield, and voltage-rated gloves to protect against arc flash hazards and electrical shock hazards.  This exposed employee(s) working at or around the circuit breaker to Arc-flash, electrical shock, and burns.
