Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:  Production Area:  On, or about, February 5, 2021, the White Model MA-25 forktruck was used to lift a mold during the set-up of Machine #26, a Reed 200 ton injection molding machine.  The forktruck had been modified to lift molds by installing an eyebolt at the tip of an inverted fork.  The estimated reduced capacity of the forktruck was calculated to be 1,367 pounds.  The modification had not been done with the permission of the manufacturer or manufacturer representation and the reduced capacity had not been marked on the forktruck.
