29 CFR 1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. a) At the Maintenance Shop, on or about 2/13/2021: The employer did not perform a hazard assessment prior to exposure when employees were permitted to operate a Roper Whitney Pexto Shear without adequate guarding to protect against injuries. This resulted in an amputation. b) At the Maintenance Shop, on or about 2/13/2021: The employer did not perform a hazard assessment prior to exposure when employees were permitted to operate an Ellis Drill Press without guarding around the rotating chuck, exposing workers to lacerations.
