29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. (a) 1616 Bellinger Street, Eau Claire, WI; At the south end of the new addition located in the sub-level of the facility, a 50 foot 12 AWG flexible extension cord used to supply power to temporary lighting had damage to the outer insulation, exposing inner wires.
