29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling in the following locations: Carpenter Shop: A receptacle box, designed for fixed installation, was wired with a black flexible cord and attached to a plug was not securely affixed to any surface. The assembly was being used as an extension cord to power a Rockwell Unisaw table saw. Employees were exposed to electrical hazards.
