The power transmission belt an pulley system was within 4 feet of the floor surface.  B) Penn Weld Inc., on or about February 18, 2021 at the address of 1057 French Street, Meadville, PA/South Side of Building:  The power transmission belt/pulley system was not guarded on the pedestal wire wheel/buffer. The power transmission belt and pulley system was within 4 feet of the floor surface.

A) Penn Weld, Inc., on or about February 18, 2021 at the address of 1057 French Street, Meadville, PA:  Shop fabricated extended forks were used on the Caterpillar Fork Lift to pick oversized pallets off of shipping trucks in the facility.  Manufacturer's prior written approval was not obtained to the use of this forklift attachments.

The data plates, capacity plates and load charts on the forklift were not changed to represent the forklift attachments effect on the forklift's capacity/stability.
