29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a. On or about February 16, 2021, an employee was pressure washing the rooftop of an apartment building without the use of conventional fall protection; therefore exposing the employee to a fall hazard.

29 CFR  1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  a. On or about February 17, 2021, an employee was working from the basket of an aerial lift to apply paint to the exterior walls of a building, without the use of a personal fall arrest system or a travel restraint system properly attached to the boom or basket; therefore exposing the employee to a fall hazard.
