29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about 02/06/2021 - 26999 Old Madison Highway 20, Madison, AL, Clean Room Area, Building #200, employees installing supply air ducts on a ceiling with six (6 )hatch access doors (46" x 31") were not protected from falling approximately 9.5 feet to the ground below.
