29 CFR 1910.132(f)(3)(iii): The employer did not retrain employees when inadequacies in the affected employee's use of PPE indicated the employee had not retained the requisite understanding: a) JBS Foods, Inc., 800 N. 8th Ave, Greeley, CO 80631: On or before February 17, 2021, the employer did not ensure that employees retained the requisite understanding or skill in that there were inadequacies in affected employee's knowledge or use of assigned PPE. During the end of shift clean-up task, an employee cleaning product from the PTC-6 conveyor belt reached for product that fell off the end after removing the blue plastic sleeve that, among other things, ensures the smock sleeves do not dangle near danger zones. The worker was caught in the drive sprockets of the conveyor. This condition resulted in a left arm amputation to the employee.

This condition exposes employees to caught-in and amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) JBS Foods, Inc., 800 N. 8th Ave, Greeley, CO 80631: On or before February 18, 2021, the employer did not guard the LR8C incline conveyor belt above entrance into the PTC3 workstation and power 90 area to protect employees from contacting the unguarded bottom (return) side of the conveyor. Employees entered and exited the PTC3 work station and power 90 area at different heights to avoid coming into contact with the ingoing nip points on the moving conveyor belt. This condition exposed employees to scalping caught-in and amputation hazards.

During the end of shift clean-up task, an employee cleaning up product from the PTC-6 conveyor belt, reached for the product that fell off the end and became caught in the drive sprockets. This condition resulted in a left arm amputation to an employee.
