29 CFR 1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident . a) On 2/17/2012, an employee of AVC General Contracting LLC was taking measurements atop the roof of three-story house at 144 Dunham Street in Staten Island NY 10309, when he fell 22 ft. from mid-point along the north edge. He was pronounced dead at the hospital the same day. The employer was aware of the employee's death on 2/17/2021; but did not report the death to OSHA. Condition(s) were noted on or about 2/18/2021.
