Energy control procedures were not developed for set-up operations, servicing, and maintenance activities on the CNC mills, lathes, and boring machines exposing employees to entanglement and struck by hazards.

29 CFR  1910.147(c)(7)(i)(A):  Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  Employees authorized to perform set up on the CNC lathes, CNC mills, and CNC boring machines were not trained as an authorized employee under the control of hazardous energy.

29 CFR  1910.178(l)(1)(ii):  Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).  Employees operating the forklifts were not trained prior to ensure they had the knowledge and skills to operate safely.

29 CFR  1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  (a)  In the Lathe Department, the #4332 HIT-460 did not have door interlocks exposing employees to potential moving parts while loading/unloading parts.    (b)  In the Lathe Department, the Mori Seiki SL-25B had the door interlock key in the actuator exposing employees to potential moving parts while loading/unloading parts.    (c)  In the Lathe Department, the  #4331 Mori Seiki SKT 25 CNC Turning Center had the door interlock key in the actuator exposing employees to potential moving parts while loading/unloading parts.   (d)  In the Lathe Department, #4330 Hyunadi Wia did not have operational door interlocks allowing the door to be open during operation and exposing employees to potential moving parts while loading/unloading parts.   (e)  In the Lathe Department, Puma 250 CNC Lathe had the door limit switch in the up position on the left side of the door exposing employees to potential moving parts while loading/unloading parts.    (f)  In the Lathe Department, the #4313 vertical boring machine Gibbing & Lewis had three carousels of tools which were not guarded exposing employees to moving parts.

29 CFR  1910.1200(g)(1):  Chemical manufacturers and importers shall obtain or develop a safety data sheet for each hazardous chemical they produce or import. Employers shall have a safety data sheet in the workplace for each hazardous chemical which they use.  The employer did not maintain the Safety Data Sheet for the Master Fluid Solutions, Master Stages Whamex, Pen Dip, and Task 2 Grime Fighter.
