29 CFR  1910.30(a)(1):29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  On or about February 23, 2021 employees required to work at heights were not provided with training to recognize and minimize fall  hazards, exposing employees to a fall hazard.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.28(b)(1)(i):29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about February 2, 2021, employees engaged in snow removal from a commercial roof were not protected from a fall, exposing the employees to a fall of approximately 30 feet.  ECS Roofing Professionals, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard ,1926.501(b)(11) which was contained in OSHA inspection number 1476805, citation number 1, item number 2 and was affirmed as a final order on July 15, 2020, with respect to a workplace located at 1 Barrington Rd. Schaumburg, IL 60194.  ECS Roofing Professionals, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard ,1926.501(b)(10) which was contained in OSHA inspection number 1470040, citation number 1, item number 1 and was affirmed as a final order on July 14, 2020, with respect to a workplace located at 5235 Prairie Stone Parkway Hoffman Estates, IL 60192.  Josh Herion Dba Ecs Roofing, Inc., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(11) which was contained in OSHA inspection number 1444224, citation number 1, item number 2 and was affirmed as a final order on March 13, 2020, with respect to a workplace located at 1801 Milwaukee Ave. Glenview, IL 60025 60192.  Josh Herion Dba Ecs Roofing, Inc., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13) which was contained in OSHA inspection number 1385181, citation number 2, item number 2 and was affirmed as a final order on April 30, 2019, with respect to a workplace located at Lot 122 Tullamore Lane Lemont, IL 60439 .  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
