29 CFR 1910.138(b): Employers did not base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified. On or about February 15, 2021, the employer did not select and provide the appropriate hand protection to employees working with wet conditions in extreme temperatures. This exposed the employees to hand injuries.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. An in-patient hospitalization affecting one (1) employee, occurring on or about February 16, 2021, was not reported to the local OSHA area office within the 24- hour reporting requirement.
