1904.39(a)(3)  The employer did not report the fatality, in-patient hospitalization, amputation, or loss of an eye using one of the following methods: (i) By telephone or in person to the OSHA Area Office that is nearest to the site of the incident. (ii) By telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742). (iii) By electronic submission using the reporting application located on OSHA's public website at www.osha.gov.   (a)  U.S. POSTAL FACILITY, 1675 CAMP HILL BY-PASS, CAMP HILL, PA - An employee sustained an injury and was admitted to the hospital on December 24, 2020, and the employer did not report the in-patient hospitalization to OSHA until December 27, 2021.  U.S. Postal Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1286249, citation number 1, item number 1,  and was affirmed as a final order on May 30, 2018, with respect to a workplace located at 1250 Smile Way, York,  Pennsylvania.  U.S. Postal Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1288391, citation number 1, item number 1, and was affirmed as a final order on June 8, 2018, with respect to a workplace located at 6450 Kings Highway, Zionsville, Pennsylvania. 29 CFR  1904.35(b)(1)(iv):  The Employer discriminated or retaliated against the employee for reporting a work-related  injury or illness:  (a)  U.S. POSTAL FACILITY, CAMP HILL, PA - At the Camp Hill, Pennsylvania, United States Postal Facility, on or about January 5, 2020 - The employer terminated the Letter Carrier because the Letter Carrier reported a work-related injury where the employee sustained injuries to the right elbow.  (b)  U.S. POSTAL FACILITY, CAMP HILL, PA  -  At the Camp Hill, Pennsylvania, United States Postal Facility, on or about February 3, 2021 - The employer terminated the Letter Carrier because the Letter Carrier reported a work-related injury where the employee sustained injuries to the left knee.  The United States Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1904.35(b)(1)(iv), which was contained in OSHA inspection number 1365472, citation number 1, item number 1 and was affirmed as a final order on February 24, 2020, with respect to a workplace located at 140 Brownsville Road, Pittsburgh, PA.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
