(b)  Prison staff / inmates engaged in operating the Port-A-Cool misting fan on the west side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical shock hazards due to wiring entering the fan electrical box no longer having outer sheath properties, thus exposing approximately 1 inch of the conductors.  (c)  Prison staff / inmates) engaged in operating the Port-A-Cool misting fan on the west side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical shock hazards due to the electrical box cover for the misting fan not being closed on one side, thus exposing the electrical wiring and components inside.  (d)  Prison staff / inmates using a 4-plex outlet box mounted on the building cylinder post on the south side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical shock hazards due to right side of the outlet box containing grounding prongs inside the receptacles.
