29 CFR  1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings:  (a)  Prison staff / inmates using the stairway steps on the east end of the Camp Building located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to fall hazards due to the deteriorating top stair landing causing a non-uniform riser height between the 3rd step.  (b)  Prison staff / inmates using the stairway steps on the west end of the Camp Building located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to fall hazards due to the  deteriorating 1st, 2nd, and 3rd stairway steps causing non-uniform riser heights between the steps.

29 CFR  1910.212(a)(5):  Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:  (a)  Prison staff / inmates engaged in operating a Port-A-Cool misting fan while processing laundry along the west side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to struck-by hazards due to a 8" x 6-1/2" opening on the front side guard of the Port-A-Cool misting fan.  (b)  Prison staff / inmates engaged in operating a Port-A-Cool misting fan while processing laundry along the west side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to struck-by hazards due to no guard being provided on the back side of the Port-A-Cool misting fan.  The opening on the back side of the Port-A-Cool misting fan was approximately 73" x 59-1/2".

29 CFR  1910.303(b)(1):Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):  (a)  Prison staff / inmates engaged in operating the Port-A-Cool misting fan on the west side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical shock hazards due to floor pump wiring no longer having outer sheath properties, thus exposing approximately 2 inches of the conductors.  (b)  Prison staff / inmates engaged in operating the Port-A-Cool misting fan on the west side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical shock hazards due to wiring entering the fan electrical box no longer having outer sheath properties, thus exposing approximately 1 inch of the conductors.  (c)  Prison staff / inmates) engaged in operating the Port-A-Cool misting fan on the west side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical shock hazards due to the electrical box cover for the misting fan not being closed on one side, thus exposing the electrical wiring and components inside.  (d)  Prison staff / inmates using a 4-plex outlet box mounted on the building cylinder post on the south side of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical shock hazards due to right side of the outlet box containing grounding prongs inside the receptacles.

The metal wrapped flexible conduit was not being used according to the manufacturer's listing / labeling instructions.

29 CFR  1910.305(g)(2)(iii):  Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:  (a)  Prison staff / inmates engaged in using the ice machine along the south wall of the laundry room located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical hazards due to outer jacket exposing approximately 1/2 inch of energized conductors at the plug end of the power cord.  (b) Prison staff / inmates engaged in using the Camp Building cafeteria located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to electrical hazards due to outer jacket of the emergency light power cord exposing 3/4" inches of energized conductors at the plug end of the power cord.

29 CFR  1960.59(a):Each agency shall provide appropriate safety and health training for employees including specialized job safety and health training appropriate to the work performed by the employee. Such training also shall inform employees of the agency occupational safety and health program, with emphasis on their rights and responsibilities:  Prison staff engaged in working  at  USP Leavenworth prison and the camp buildings located at 1300 Metropolitan Avenue, Leavenworth, Kansas were exposed to health hazards due to insufficient COVID-19 policy training.  The employer did not verify that all staff members read the COVID-19 policies emailed or placed on the prison intranet site, thus the lack of knowledge / understanding to prevent exposure to the coronavirus.
