29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) At 8159 Collecton Drive South (Ground Level): On or about February 23, 2021, the employer did not ensure that hard hats were used by employee(s) on the ground while worker(s) approximately eighteen (18) feet above were performing roofing activities; exposing employee(s) to struck-by hazards from overhead falling/flying materials and equipment.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) At 8159 Collecton Drive South (Roof Area): On or about February 23, 2021; the employer required his employees to perform roofing activities without fall protection; exposing employees to fall hazards of approximately eighteen feet (18') from the eave to the concrete sidewalk and ground below.
