On or about 2/24/2021, Mid-South Steel Products, Inc. did not conduct periodic inspections on their authorized employees or for several machines in the plant to include, but not limited to a Woods Rolls # 7 Machine.

29 CFR 1910.303(b)(1):Examination. Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees. On or about 2/24/2021, Mid-South Steel Products, Inc. did not examine the electric equipment to ensure it was free from recognized hazards that are likely to cause death or serious physical harm to employees. The following equipment had electrical hazards: a) A pedestal control for a Baileigh Industrial Plate Bending Machine, Model PR-609/Serial # 47171, had damaged/missing conduit near the attachment point on the base of the pedestal. The conductors/wires were not provided with strain relief or adequately protected from accidental contact. b) The insulation/outer jacket on the electrical cord connecting the control pendant to the Webb Tank Turning Machine was damaged/missing. The wire conductors were exposed and not protected from accidental contact.
