On or about February 24, 2021 at jobsite located in 384 S. Prairie Ave., Elmhurst, IL 60126, employees were exposed to fall hazards of approximately 25 feet while engaged in residential roofing activities. Employees were observed not using fall protection correctly when the lifeline lanyard was connected to the front of the harness while performing roofing operations.

Prairie Ave., Elmhurst, IL 60126, an employee was observed climbing a ladder with material in his hands and not maintaining three points of contact when ascending the ladder. This exposed employee to fall hazards when using the extension ladders improperly.
