29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   On the north side of the commercial building, employees engaged in roofing work on a 5:12 pitch roof without fall protection were exposed to unprotected fall hazards of approximately 11 feet to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.     On the north side of the commercial building, employees engaged in roofing work used an extension ladder that did not have the side rails extended three feet over the upper landing surface  exposing employees to potential fall hazards greater than six feet.  Misael Cortez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1294529, citation number 01, item number 004 and was affirmed as a final order on March 20, 2018, with respect to a workplace located at 7575 Chaucer Place, Dallas, Texas 75237.

29 CFR  1926.503(a)(2):  At this job site, the employer had not assured that each employee has been trained, as necessary, by a competent person qualified in fall hazards.     On the construction site, the employer did not ensure each employee was trained to recognize and correct safety hazards associated with roofing work such as fall hazards while working over six fee without fall protection and personal protection equipment required on construction sites.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:   On the construction site, the employer did not ensure each employee was trained to recognize and correct safety hazards associated with ladder safety and how to recognize and eliminate the hazards where employees engaged in roofing work used an extension ladder that did not have the side rails extended three feet over the upper landing surface  exposing employees to potential fall hazards greater than six feet.
