29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) On February 24, 2021; the controlling employer, Visser Building Co., L.C. did not have an effective system in place to correct hazards to which their subcontractors were exposed.  These hazards include but may not be limited to fall hazards up to 16 feet, on and around the interior spiral stairway.     b) On February 24, 2021; the controlling employer, Visser Building Co., L.C. did not have an effective system in place to correct hazards to which their subcontractors were exposed.  These hazards include but may not be limited to maintaining proper machine guarding on table saws used to cut finish woodwork.  Note) Abatement certification and documentation are required for this item.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On February 24, 2021; the controlling employer, Visser Building Co., L.C. did not ensure sub-contractors protected their employees from fall hazards.  Sub-contractor employees performed work, including but not limited to finish carpentry work, on or near the interior spiral stairway landing without a fall protection system in place.  This exposed the employee(s) to a 16 foot fall hazard the floor below.    Note) Corrected during inspection.

29 CFR  1926.1052(c)(4): Mid rails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not provided between the top rail of the stair rail system and the stairway steps:  a) Interior spiral stairway:  On February 24, 2021; the controlling employer, Visser Building Co., L.C. did not ensure sub-contractors protected their employees from fall hazards while working on the interior spiral stairway.  Top rails and mid rails were missing at various locations along the stair rail system.    Note) Corrected during inspection.
