On or about February 25, 2021, and at times prior thereto, employee operating Werner extension ladder that did not extend at least three feet above the upper landing surface, exposing employee to a fall hazard.

29 CFR  1910.23(b)(10):(10) Any ladder with structural or other defects is immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced.  On or about February 25, 2021, and at times prior thereto, employees were utilizing a Werner extension ladder that was damaged. The ladder was missing the entire sliding extension ladder and the footings of the remaining ladder were damaged, exposing employees to a fall hazard.
