(a) San Clemente Island SCORE Berthing REWS-T-108: The south stairway landing was 6.5 inches away from the doorway exposing a gap to the ground level. Employees potentially exposed to trips.  (b) San Clemente Island SCORE Berthing REWS-T-107: The north stairway landing was 4.5 inches away from the doorway exposing a gap to the ground level. Employees potentially exposed to trips.  (c) San Clemente Island SCORE Berthing REWS-T-100: The south stairway landing was 7 inches away from the doorway exposing a gap to the ground level. Employees potentially exposed to trips.

29 CFR 1910.132(d)(2):The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  San Clemente Island SCORE Berthing REWS-T-100 through108: Employees removed dead rodents and their droppings from inside the trailers on a frequent basis. The employer had not certified that a hazard assessment had been completed for this task. Employees potentially exposed to rodent borne diseases such as Hantavirus.

Employees removed dead rodents and their droppings from inside Trailers on a frequent basis. The employer did not have an effective extermination program. Employees potentially exposed to rodent borne diseases such as Hantavirus.

29 CFR 1910.142(a)(1):Temporary labor camp site(s) were not graded, ditched, and rendered free from depressions in which water could have become a nuisance:  (a) San Clemente Island SCORE Berthing REWS-T-107: At the bottom of the stairway a raised wood plank was placed where water accumulates after rain. The wood plank was placed there so employees did not have to walk through the water and mud. The employer has not ensured the ground was graded, ditched, and rendered free from depressions in which water could have become a nuisance. Employees potentially exposed to slips, trips, and falls.  (b) San Clemente Island SCORE Berthing REWS-T-105 and 106: At the bottom of the two adjoining stairways a raised wood plank was placed where water accumulates after rain. The wood plank was placed there so employees did not have to walk through the water and mud. The employer has not ensured the ground was graded, ditched, and rendered free from depressions in which water could have become a nuisance. Employees potentially exposed to slips, trips, falls.

29 CFR  1910.304(b)(3)(i): All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops did not have ground-fault circuit-interrupter protection for personnel.  San Clemente Island SCORE Berthing REWS-T-100 through108: Employees use the sinks in the bathrooms and the electrical outlets in these areas are not protected by ground-fault circuit-interrupters (GFCI). The employer did not ensure the bathroom outlets were protected by GFCIs. Employees potentially exposed to electric shock hazards.

29 CFR 1910.142(k)(1): Adequate first aid facilities approved by a health authority were not maintained and made available in every labor camp for the emergency treatment of injured persons:   San Clemente Island SCORE Berthing REWS-T-100 through108: The small first aid kits in the trailers were not maintained with adequate supplies. First aid kits were missing supplies such as; gloves, band-aids, and items were expired such as antiseptic towelettes and creams/wipes. Employees potentially exposed to infections.
