29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about February 25, 2021 the employer failed to ensure that specific procedures for the control of hazardous energy during servicing and maintaining of equipment in the facility were in place. Employees are tasked with servicing and maintaining of equipment including but not limited to saws, wood milling machines, routers, and other wood working machines. Hazardous energies present on the equipment includes pneumatic, mechanical, and electrical. Procedures were not in place with sufficient detail and adequate guidance for an authorized employee(s) to clearly understand how to safely and effectively utilize hazardous energy control measures for the particular machine or piece of equipment being serviced and/or maintained.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about February 25, 2021 the employer failed to ensure that a periodic inspection of the machine specific procedures for equipment serviced and maintained in the facility had taken place. Employees are tasked with servicing and maintaining of equipment including but not limited to saws, wood milling machines, routers, and other wood working machines. Hazardous energies present on the equipment includes pneumatic, mechanical, and electrical.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about February 25, 2021 the employer failed to ensure that effective machine guarding was in place on the automatic wood routing machine (Maypro Cablematic #C832). Barrier guarding was not in place on the non-operator side allowing employees to enter the cell area where the movement of the machine and point of operation of the router was not enclosed.

29 CFR  1910.333(a)(1): Live parts that the employee was exposed to were not de-energized before the work began in or near them.    On or about February 25, 2021 the employer failed to ensure that the electrical components of the hydraulic press (Hidrogarne model CD45/65F) in the Nail room were deenergized prior to employees working near the exposed electrical conductors. The control panel labelled as containing 480v equipment was opened and the main control switch operated inside the panel by employees.
