29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On and about June 2, 2021, employees were changing saw blades on a table type chop saw, and a Brewco Model Number BF20 pallet saws, and the employer did not establish a program consisting of energy control procedures, employee training, and periodic inspections of either of these two machines.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about June 2, 2021, employees were exposed to machine hazards associated with rotating parts of circular saw blades while changing the blade on a red table top chop saw. The employer failed to implement energy control application steps as the machine was not isolated from electrical energy sources [(d)(3)]. b) On or about June 2, 2021, 2021, employees were exposed to machine hazards associated with rotating parts of circular saw blades while changing the blades on a Brewco Resaw Machine, Model Number BF20, serial number B43158. The employer failed to implement energy control application steps as the machine was not isolated from electrical and pneumatic energy sources [(d)(3)]. In these instances, LOTO devices, such as locks, were not applied to the energy sources [(d)(4)]. As a result, the remaining applicable energy control elements regarding the dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
