29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about February 18, 2021, for the employees exposed to Hydrogen Sulfide and hydrocarbon vapors where the employer failed to ensure that energy sources were isolated, the energy released, and the equipment locked out, prior to repairs of a natural gas line feeding the flare pilot of an oil production heater/treater system located on a wellsite in the vicinity of Highway 55 & 44th Street NW in Charlson, North Dakota.
