On or about February 25, 2021, at the above addressed jobsite, an employee conducting carpentry operations on a scaffold was improperly tied off by wrapping his lanyard around a roof truss and attaching the snaphook to the lanyard strap, thereby exposing the employee to fall hazards of seventeen (17) feet.
