29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a.  On or about February 25, 2021, at the above addressed jobsite, an employee was exposed to falls while performing carpentry operations from a homemade wooden scaffold, seventeen (17) feet above the ground, that did not have the working level fully planked.

On or about February 25, 2021, at the above addressed jobsite, an employee performing carpentry operations on a house from a homemade wooden scaffold, at seventeen (17) feet above the ground, was exposed to fall hazards due to the lack of guardrails or wearing a personal fall arrest system (PFAS).

29 CFR 1926.502(d)(6)(ii): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  a.  On or about February 25, 2021, at the above addressed jobsite, an employee conducting carpentry operations on a scaffold was improperly tied off by wrapping his lanyard around a roof truss and attaching the snaphook to the lanyard strap, thereby exposing the employee to fall hazards of seventeen (17) feet.
