29 CFR 1910.178(q)(1):Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: (a) Shipping Department: On or about 2/17/2021 and at times prior, the employer exposed powered industrial truck operators who transport material throughout the facility to electrical hazards in that battery cables on the #20 Caterpillar powered industrial truck had exposed wiring and was not taken out of service.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: (a) Receiving Department - Battery Charging Area : On or about 2/17/2021 and at times prior, the employer exposed forklift operators to electrical hazards in that the battery charging socket connector was damaged and the battery cable was pulled out with exposed wiring.

29 CFR 1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a)Receiving Department Battery Charging Area: On or about 2/17/2021, and at times prior, the employer exposed powered industrial truck operators to electrical hazards, in that the plug-in cable for the Green Diamond battery charger did not have strain relief to prevent tension to the terminal screws.
