29 CFR 1910.178(l) (4) (ii) (A): Refresher training in relevant topics was not provided to the operator that has been observed to operate the vehicle in an unsafe manner: a. 560 Horizon Drive, Suwanee, GA (Warehouse Area) - Employees were exposed to struck-by hazard. An employee/designated forklift operator that was involved in accident and/or unsafe incident which occurred on February 09, 2021, such as, loading and transporting a pallet, loaded with boxes using a cherry pick powered industrial truck. The employee was not given refresher training in the relevant topic, on or about 02/09/2021.
