29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Job site, 2385 Saxonburg Blvd, Cheswick, PA 15024, on or about February 26th 2021, employees in the process of installing a new roof on a commercial building were exposed to fall hazards of approximately 15 feet to the ground below. Employees were working on a 6/12 pitch roof, no harnesses, guard rails, safety nets or any other fall protection was in use.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards: a) Job site, 2385 Saxonburg Blvd, Cheswick, PA 15024, on or about February 26, 2021, employees in the process of installing a new roof on a commercial building were exposed to fall hazards of approximately 15 feet to the ground below. Employer did not provide employees any type of fall protection training for the hazards associated when working from heights.
