29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  Employees engaged in work activities at the locations listed below, were exposed to trip and fall hazards in that protruding objects, unlevel surfaces, and ice were not maintained, causing employee falls to the same level resulting in injuries.  A) Meat Freezer - inside freezer walking/working surface concrete is broken, leaving concrete surface unlevel with some concrete elevations approximately 4 and 1/2 inches in       height.  Metal plate leading to meat freezer door is unlevel with an elevation approximately 3 and 1/2 inches above concrete ramp.  Ice accumulation below freezer on the west       wall.  B) Bakery / Deli Department - At the old mixer location on the South wall, rebar or concrete anchor bolts are protruding from floor causing trip hazards.  The protruding objects are        approximately 22 inches apart, and 1/2 inch in height.  C) South register - Located on the floor, tile with a metal lip in a circle, has metal lip protruding approximately a 1/2 inch causing a trip hazard.

29 CFR  1910.36(d)(1):Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  Employees working in the meat freezer located in the back store room were exposed to hypothermia, frost bite, and death in that the freezer door had no way to open from inside the freezer when the door was closed.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees working in the back store room using the Piqua Series Baler 40 were exposed to amputation, struck-by and caught-in hazards, in the instances listed below:  A) The interlock system had been defeated to allow for the baler to run with the steel doors in an open position and not having to be securely locked, causing employees to be exposed to struck-by hazards.   B) The gate had a hole in the mesh metal guard, approximate 5 inches in height and 4 inches in width, exposing employees to caught-in and amputation hazards.
