29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    (a)	Cardinal Glass Industries, Inc. at 999 N. Van Buren Ave., Loveland, CO 80537:  On and before February 26, 2021 the employer permitted an oxygen and an acetylene tank to be stored adjacent to each other on a cutting torch cart in the maintenance area.  The tanks were not capped, regulators and hoses still connected with valves turned off, but the cylinders were securely fixed to the cart.  This condition potentially exposed employees to a fire and explosion hazard.

29 CFR 1910.101(b): Section 3.3.1, Compressed Gas Association Pamphlet P1 1965, as adopted by 29 CFR 1910.101(b): Compressed gas cylinder(s) should be stored in accordance with all local, state and municipal regulations and in accordance with appropriate standards of the Compressed Gas Association and the National Fire Protection Association:  (a)	Cardinal Glass Industries, Inc. at 999 N. Van Buren Ave., Loveland, CO 80537:  On and before February 26, 2021 the employer did not ensure that compressed gas cylinders were secured from being knocked over in that an oxygen cylinder in the maintenance area was not secured from being knocked over. This condition potentially exposed employees to fire, explosion, or struck-by hazards.

29 CFR  1910.1200(f)(6):Workplace labeling. Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either:  29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either: (i) The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, (ii) Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a)	Cardinal Glass Industries, Inc. at 999 N. Van Buren Ave., Loveland, CO 80537:  On and before February 26, 2021 the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked to convey the hazards represented by the employee use of chemicals such as, but not limited to:  (1)    Unleaded gasoline in the Maintenance Shop (2)    Two stroke engine gasoline in the Maintenance Shop (3)    Magnudraw Vanishing Oil in the Laminated Cutting Area  This condition exposed employees to chemical hazards.
