29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: 1) On March 1, 2021, and at prior times, RC Andersen, L.L.C., as the controlling employer, did not ensure that employees of its subcontractors were protected from tripping in or stepping into holes in the following instances: a) Covers were not placed on floor holes around the right hand side or front of ductwork in 6 locations on the 4th floor. b) Covers were not placed on a floor hole that extended the length of the 4th floor.
