On or about March 2, 2021, at the above addressed jobsite, an employee who did not receive training to identify fall hazards was exposed to falling 18 feet to ground level from a roof. b. On or about March 2, 2021, at the above addressed jobsite, an employee accessed a ladder that was not at least 3 feet over the landing surface and had not received training on ladder safety, exposing the employee to a fall hazard of approximately 18 feet to ground level.
