29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):    On or about March 2, 2021, at the above addressed jobsite, an employee  was exposed to a fall hazard of 18 feet to ground level when leaning over the edge of a flat roof and not protected from falling.

29 CFR  1910.30(a)(3)(i):The employer did not train each employee in the nature of the fall hazards in the work area and how to recognize them:  a. On or about March 2, 2021, at the above addressed jobsite, an employee who did not receive training to identify fall hazards was exposed to falling 18 feet to ground level from a roof.     b. On or about March 2, 2021, at the above addressed jobsite, an employee accessed a ladder that was not at least 3 feet over the landing surface and had not received training on ladder safety, exposing the employee to a fall hazard of approximately 18 feet to ground level.

29 CFR 1910.23(d)(4): The employer did not ensure that the side rails of through or sidestep ladders extend at least 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder:   On or about March 2, 2021, at the above addressed jobsite, an employee accessed a ladder that was not at least 3 feet over the landing surface, exposing the employee to a fall hazard of approximately 18 feet to ground level.
