29 CFR  OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and struck-by hazards:   a) On March 3, 2021 an employee was exposed to fall hazards and struck-by/run over hazards while traveling on and being lifted from the forks of a HYSTER 50, Model: H50FT, Serial#: L177B25080F.  The employee was exposed to a fall hazard of approximately 5 to 7 feet while being transported from the tire rack to a storage trailer to unload tires.  NOTE) Abatement certification AND supporting documentation are required for this item.
