29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: a. At the roof eave at 116 Hydrilla Court in Saint Augustine, Florida: On or about March 3, 2021, two framing employees were exposed to an 11 feet fall hazard in that they used the top and top steps of two fiberglass A-frame stepladders to access the 11 feet high roof eave.

29 CFR 1926.416(e)(1):Worn or frayed electric cords or cables were used: a. At the front left corner of the house at the roof sheathing board stack at 116Hydrilla Court in Saint Augustine Florida: On or about March 3, 2021, an extension cord connecting the Makita circular saw to the generator had a torn outer insulator jacket. The extension cord was run from the generator to the Makita saw. The outer insulating jacket of the cord was torn where it entered the plug from the strain of a bend where the cord was tied in a knot.
