29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  D&R Roofing, LLC at the worksite located at 1450 Caymus Ct. (Unit #44) in Avon, Ohio: On or about March 3, 2021, the employer did not ensure that employees were trained to recognize appropriate use of fall protection. Employees performing roofing activities on a residential structure were exposed to fall hazards of up to approximately 35 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  D&R Roofing, LLC at the worksite located at 1450 Caymus Ct. (Unit #44) in Avon, Ohio: On or about March 3, 2021, the employer did not ensure employees were protected from falls of approximately 35 feet while performing roofing operations on a residential structure.   D&R Roofing, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1445331,  citation number 1, item number 1 and was affirmed as a final order on November 20, 2019, with respect to a workplace located at 1466 Jude Ave., Streetsboro, OH 44241.  D&R Roofing, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1365229, citation number 1, item number 1 and was affirmed as a final order on January 30, 2019, with respect to a workplace located at 5487 Imagine Ln., Medina, OH 44256.  D&R Roofing, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1312188, citation number 1, item number 2 and was affirmed as a final order on  June 6, 2018  with respect to a workplace located at S/L 50 Nash Ln., North Ridgeville, OH 44039.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
