29 CFR  1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  On March 2, 2021, located inside an active parking lot, an employee was exposed to a fall hazard installing LED lights working from inside articulated boom lift not wearing a harness with a lanyard attached to the boom or basket. Potential fall height measured to be 25 feet.

29 CFR  1926.454(a):The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  On March 2, 2021, located inside an active parking lot, an employee was operating a Genie Z-34/22 N Articulated Boom Lift in that the employee had not receive training to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.
