29 CFR  1926.451(f)(6):Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):  Rear of house:  Employees were using ladder jack scaffolding approximately two (2) inches from an insulated powerline with less than 300 volts where a minimum of three (3) feet clearance is required.

29 CFR  1926.452(k)(5):Scaffold platforms were bridged on to another.  Rear of house:  Employees were working from ladder jack scaffolding that had two planks bridged exposing them to falls of approximately ten (10) feet to the ground below.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Rear of house:  Employees were engaged in replacing an asphalt shingle roof without fall protection exposing them to falls of greater than ten (10) feet to the ground below.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    Back of house:  A Werner sixteen (16) foot aluminum extension ladder used to access the roof was not secured to prevent movement or extend three rungs above the landing exposing employees to falls of greater than ten (10) feet to the debris covered ground below.
