29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: At this site, an employee applied stucco while on an aerial lift at heights greater than 6 feet. The employee was not wearing a fall protection belt/lanyard at the time of the operation. This exposed the employee to fall hazards.

29 CFR 1926.503(c)(3):Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. At this worksite, an employee worked from an aerial lift with no fall protection at heights greater than 6 feet. The employee stated that he had not been trained by the employer on fall protection. This exposed the employee to fall hazards.
