The employees did not recognize the hazards associated with working from an elevated location of 25 ft. without the benefit of fall protection.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder. a) OZ Construction LLC on or about March 4, 2021, at the address of 1050 Medical Center Pkwy, Maumee, OH: The employer did not ensure the side rails of 30 ft. aluminum extension ladders used onsite were extended at least three feet above the landing surface. Seven employees were exposed to a near 25' fall to the level below while performing roofing activities using the ladder to access the south side of the commercial structure roof.
