At worksite located at 1275 N Golden Clinton, MO., Forklift operator employees in the boat shipping area were exposed to serious struck by injuries from the Heli H2000 Series Forklift, Serial Number 230351L5876 that was in operation and forklift mast pulley was defective and in need of repair.
