Located at 212 Arlington Drive, Pickerington, Ohio: On or about March 5, 2021, employees were using an extension ladder to access a pump jack scaffolding system that was placed against the edge of a pitched roof. The pitch of the roof did not provide a level surface for the ladder's upper support, thereby exposing employees to a fall hazard while using the ladder.
