29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected by proper fall protection. 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a). On or about March 5, 2021 on the south side of the construction site, employee was on roof with unprotected sides without proper fall protection arrest at heights upwards of ten (10) feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about March 5, 2021 on the south side of the construction site, employee was utilizing a Werner 24 foot extension ladder that was not extended three (3) feet above the upper landing surface and not secured at any point to access the roof area.
