29 CFR  OSH ACT of 1970 Section (5)(a)(1):Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were not protected from the fall and struck-by hazard posed by an inadequately engaged shackle pin.   Location:  Vacuum attachment on job site  A shackle pin used to secure a safety sling was not fully engaged.

Location:  Job site  The Working Load Limits for the slings used to hoist the vacuum attachment were illegible.
