29 CFR  1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  a) Worksite, residential structure yard:  On or about March 5, 2021, employees were exposed to tripping hazards from debris and construction materials that was not kept clear at the worksite.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  a) Worksite, residential attic floor, south corner:   On or about March 5, 2021, employees were exposed to overhead hazards while working directly below the installation of ridge beams and rafters, without the use of head protection.  Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a) Worksite, residential structure attic floor:   On or about March 5, 2021, employees were exposed to falling approximately 30 feet from the edge of a residential structure attic floor while framing the roof.  b) Worksite, residential structure, second floor:  On or about March 5, 2021, an employee was exposed to falling approximately 20 feet from the edge of a residential structure while framing windows on the second floor.  Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
