Hoover Wichita KS, employee(s) engaged in commercial roofing activities were exposed to fall hazards of approximately 8.5 feet in that the scaffold was more that 24 inches from the surface.

Hoover Wichita KS, employees engaged in commercial roofing activities were exposed to fall hazards of approximately 10.5 feet from the working surface to the ground in that fall protection was not utilized.

Hoover Wichita KS, employees engaged in commercial roofing activities were exposed to fall hazards of approximately 10.5 feet from the working surface to the ground below in that fall protection training was not provided.
